Reports to:                  Team Leader and/or Residential Coordinator

 

Job Summary:          
Under the direction of the Team Leader and/or Residential Coordinator and within the limits of the approved policies and procedures of the Agency and any outside regulating body including Licensing, be responsible for assisting with the day to day operations of programs, and housing for the benefit, support and enhancement of life quality for individuals supported in their homes.

 

Community Support Workers are to model and contribute to the essence of home allowing opportunity for personal choice and preference for all individuals supported in the program.

 

Knowledgeable and supportive of the mission and philosophy, policies and procedures of Shelburne Association Supporting Inclusion. Endeavouring at all times to promote the independence and individuality of each person supported.

Qualifications:
Residential Vocational Workers Course or its equivalent. Must meet minimum training standards of Department of Community Services.

Relevant job skills include:

·        Experience supporting individuals with disabilities and/or behavioural challenges

·        Ability to provide guidance and training in life skills

·        Ability to implement Individual Support Plans and monitor progress towards goals

·        Behaviour Management techniques

·        Recreation Leadership

·        Ability to interact effectively including oral and written communications

·        Physically able to carry out duties

·        A pleasant, positive nurturing personality who identifies the importance of the essence of home and enhancement of life

Required Documentation: Current Certificates in minimum standards, Motor Vehicle Driving Abstract, Criminal Records Check, Valid NS Class 5 unrestricted Drivers License

 

Probationary Period: Hours equivalent to nine months for full-time and 1440 hours for part-time. 

 

Position Duties and Responsibilities:

The following description of duties applies generally to all Community Support Workers employed by Shelburne Association Supporting Inclusion. This description is representative of, but does not necessarily limit, the duties of a CSW on a specific shift within a specific program. The actual duties and practices relating to any given shift within a home are defined by the Team Leader and Residential Coordinator in relation to the specific needs of clients and program protocols in place for these clients.

 

Broad position expectations include the following:

·        To interact effectively and respectfully with the clients in their home and in the community

·        To support clients in knowing their rights and responsibilities as adult citizens

·        To support clients in the development of relationships based on individual preference and choice

·        To support and promote the relationships with family and friends while maintaining a family culture within the home

·        To support and promote friendly and effective working relationships internally and with community partners including but not limited to: Vocational Program staff, Doctors, Educational agencies, etc.

·        To support and promote community based opportunities according to individual preferences

·        To contribute in a positive manner at team meetings and special events such as parties, vacations, and other celebrations

·        To avail self of additional relevant training according to personal interests and or management’s recommendations

·        To assist with the orientation and training of new staff to ensure competent, confident employees thus ensuring the health, safety and well being for the individuals we support

 

More specific job duties include:

 

Personal Care
·        To assist individuals in the least intrusive manner possible and specific to individual needs in bathing, dressing, eating, toileting, administering medications and all other aspects of personal hygiene and care as required. Ensuring that the individual is meeting their greatest potential for independence.

·        To assist clients in attending professional appointments, purchasing clothing and other items as necessary

 

 

Social/Recreational

·        To organize/coordinate, support and participate in recreational and social opportunities with individuals based on their personal preference and choices. 

·        To support the individual with exploring new avenues for social, recreational, spiritual and artistic pursuits within their community.

 

Communication
·        To ensure all communication within the workplace is maintained in a manner which enhances professionalism, and confidentiality, and reflects respect and cooperation with co-workers, management and other members of the team.

·        To ensure all written documentation is professional, accurate and legible.

·        To be responsible for the completion of individual support plans and reports, daily logs, medication systems, appointment records, incident reports, etc. within designated time frames.

·        To attend and participate in meetings while ensuring relevant preparation and input.

·        To be aware of recent developments within the work place at the beginning of each shift, i.e. reading daily logs, verbal exchange of information, and so forth.

 

Household
·        To support clients with menu planning, grocery shopping, and meal preparation as required

·        To support clients with all general household duties related to the upkeep and minor repair of a home and property to maintain the best possible standards of cleanliness and maintenance


Other

·        To assist with in-house orientations, and training of new employees, volunteers and students.

·        To occasionally be available on days off in case of emergencies.

·        To know and be able to implement the Emergency Evacuation Plan and other safety measures as needed.

·        To attend and participate in identified training specific to the needs of the residents.

·        To work rotating shifts.

·        To be accountable for spending and recording of house and resident monies within prescribed budgets and procedures.

·        To perform duties of the job independently as required (taking clients shopping, to medical appointments, etc)

·        To assist with the regular maintenance (cleaning, monitoring fluids, etc.) of the association’s vehicles.

·        Other related duties as required.
